    Topic Modelling of Everyday Sexism Project Entries

    The Everyday Sexism Project documents everyday examples of sexism reported by volunteer contributors from all around the world. It collected 100,000 entries in 13+ languages within the first 3 years of its existence. The content of reports in various languages submitted to Everyday Sexism is a valuable source of crowdsourced information with great potential for feminist and gender studies. In this paper, we take a computational approach to analyze the content of reports. We use topic-modelling techniques to extract emerging topics and concepts from the reports, and to map the semantic relations between those topics. The resulting picture closely resembles and adds to that arrived at through qualitative analysis, showing that this form of topic modeling could be useful for sifting through datasets that had not previously been subject to any analysis. More precisely, we come up with a map of topics for two different resolutions of our topic model and discuss the connection between the identified topics. In the low resolution picture, for instance, we found Public space/Street, Online, Work related/Office, Transport, School, Media harassment, and Domestic abuse. Among these, the strongest connection is between Public space/Street harassment and Domestic abuse and sexism in personal relationships.The strength of the relationships between topics illustrates the fluid and ubiquitous nature of sexism, with no single experience being unrelated to another.Comment: preprint, under revie

    Trace Element Analysis of Rutile and Zr-in-Rutile Thermometry for Southern Appalachian Pelitic Schists

    Over the past decade, geochronology studies in the southern Appalachians have focused on zircon, monazite, and mica ages to confirm the occurrence of a major Taconic event (~460‐450 Ma) affecting the Western and Central Blue Ridge, followed by a younger NeoAcadian (~360‐345 Ma) event affecting the Eastern Blue Ridge and Piedmont. Peak conditions of granulite facies metamorphism are estimated at ~850°C (garnet‐biotite) and 7‐9 kbar (GASP) for sillimanite schists at Winding Stair Gap (WSG), but thermobarometric studies of metasedimentary rocks in the region are limited and consequently Pressure‐Temperature‐time‐Deformation (P‐T‐t‐D) paths are poorly understood. Many details of the orogenic processes in the region remain unanswered, such as the depth of burial, rates of burial, cooling, and exhumation of various terranes, and variation of ages along strike. Rutile provides a robust, high‐temperature U‐Pb geochronometer that forms during metamorphic reactions and is a key phase for applying several thermobarometers in the determination of P‐T conditions. This study focuses on electron microprobe analysis of rutile‐bearing metapelitic schists in the Blue Ridge to obtain major and some trace element data and the generation of temperature constraints for the orogen. Initial sampling indicates that rutile‐bearing schists are somewhat sparse across the orogen, but have been confirmed in the Great Smoky Group of the Western Blue Ridge as well as in the Cartoogechaye and Cowrock terranes of the Central Blue Ridge, all of which were metamorphosed to high grades during the Taconic Orogeny. Rutile is found in both kyanite‐ and sillimanitegrade rocks, primarily as a matrix phase associated with biotite. Common assemblages in these rocks include garnet, biotite, muscovite, quartz, opaques, plagioclase, ±kyanite, ±sillimanite, and ±staurolite. Zirconium‐in‐rutile thermometry performed on the samples generated varying results based on sample location. The thin sections from the Western Blue Ridge were consistent with one another and comparable to previously published temperature estimates of ~600°C, indicating method consistency. However, the samples of Shooting Creek schist from near the Appalachian Trail generated temperatures that were lower than expected, especially since the rutile grains exist as inclusions in the garnet and should represent an earlier, higher‐temperature assemblage. Finally, the temperatures for rocks near Winding Stair Gap were much lower than the published peak condition estimates for the area, possibly supporting similar findings from Chen et al. (2007) that temperature estimates may be consistently too low at extreme temperatures and/or pressures due to thermometer resetting

    Spent Nuclear Fuel—Waste or Resource? The Potential of Strategic Materials Recovery during Recycle for Sustainability and Advanced Waste Management

    Nuclear fuel is both the densest form of energy in its virgin state and, once used, one of the most hazardous materials known to humankind. Though commonly viewed as a waste—with over 300,000 tons stored worldwide and an additional 7–11,000 tons accumulating annually—spent nuclear fuel (SNF) represents a significant potential source of scarce, valuable strategic materials. Beyond the major (U and Pu) and minor (Np, Am, and Cm) actinides, which can be used to generate further energy, resources including the rare earth elements (Y, La, Ce, Pr, Nd, Pm, Sm, Eu, Gd, and Tb), platinum group metals, (Ru, Rh, Pd, and Ag), noble gases (He, Kr, and Xe), and a range of isotopes useful for medical and energy generation purposes are also produced during fission. One reason for the accumulation of so much SNF is the low uptake of SNF recycle (or reprocessing), primarily due to the high capital and operational costs alongside concerns regarding proliferation and wastes generated. This study will highlight the predominantly overlooked potential for the recovery of strategic materials from SNF, which may offset costs and facilitate advanced waste management techniques for minimised waste volumes, thus increasing the sustainability of the nuclear fuel cycle on the path towards Net Zero. Potential challenges in the implementation of this concept will also be identified

    Reconstructing historical patterns of primary production in Puget Sound using growth increment data from shells of long-lived geoducks (Panopea generosa)

    Bottom-up hypotheses predict that changes in primary production affect marine survival of species like Pacific salmon. Long term records of primary production would provide important data to test these predictions. However, direct observations of primary production (in situ fluorometers, water chemistry, and satellite observations of color back-scatter) have relatively short time series (\u3c 30 years). We investigated whether growth increments of geoduck clams (Panopea generosa) are correlated with primary production in different sub-basins of greater Puget Sound. Geoduck are long-lived (older specimens live \u3e100 years), widely distributed throughout the Salish Sea, and deposit annual growth rings in their shells. Shell samples from aged geoducks were by the Washington Department of Fish and Wildlife in four sub-basins within greater Puget Sound (Strait of Jan de Fuca, Southern Strait of Georgia, South Puget Sound, and Central Basin). Geoduck shells from Saratoga passage were provided by the Tulalip Tribe. Using growth indices, the known correlation of growth indices with sea surface temperature and other long-term measurements, and existing basin-level records of temperature and primary production, we modeled historical patterns of primary production in different regions of greater Puget Sound. Analyses show that the relationship between geoduck growth, temperature, and primary production varies between sub-basins, and stable isotope analysis suggests that geoducks may be more than just primary consumers. These issues make reconstruction of a historical record of primary production from growth increments challenging. Nevertheless, analyses suggest that residual growth (after accounting for temperature variation) can explain variation in annual marine survival of local coho and chinook salmon stocks. This indicates the method has promise for retrospective hypothesis testing

    Selective decontamination of the digestive tract in critically ill patients treated in intensive care units: a mixed-methods feasibility study (the SuDDICU study)

    Background: Hospital-acquired infections (HAIs) are a major cause of morbidity and mortality. Critically ill patients in intensive care units (ICUs) are particularly susceptible to these infections. One intervention that has gained much attention in reducing HAIs is selective decontamination of the digestive tract (SDD). SDD involves the application of topical non-absorbable antibiotics to the oropharynx and stomach and a short course of intravenous (i.v.) antibiotics. SDD may reduce infections and improve mortality, but has not been widely adopted in the UK or internationally. Hence, there is a need to identify the reasons for low uptake and whether or not further clinical research is needed before wider implementation would be considered appropriate. Objectives: The project objectives were to (1) identify and describe the SDD intervention, (2) identify views about the evidence base, (3) identify acceptability of further research and (4) identify feasibility of further randomised controlled trials (RCTs). Design : A four-stage approach involving (1) case studies of two ICUs in which SDD is delivered including observations, interviews and documentary analysis, (2) a three-round Delphi study for in-depth investigation of clinicians' views, including semi-structured interviews and two iterations of questionnaires with structured feedback, (3) a nationwide online survey of consultants in intensive care medicine and clinical microbiology and (4) semistructured interviews with international clinical triallists to identify the feasibility of further research. Setting : Case studies were set in two UK ICUs. Other stages of this research were conducted by telephone and online with NHS staff working in ICUs. Participants : (1) Staff involved in SDD adoption or delivery in two UK ICUs, (2) ICU experts (intensive care consultants, clinical microbiologists, hospital pharmacists and ICU clinical leads), (3) all intensive care consultants and clinical microbiologists in the UK with responsibility for patients in ICUs were invited and (4) international triallists, selected from their research profiles in intensive care, clinical trials and/or implementation trials. Interventions : SDD involves the application of topical non-absorbable antibiotics to the oropharynx and stomach and a short course of i.v. antibiotics. Main outcome measures: Levels of support for, or opposition to, SDD in UK ICUs; views about the SDD evidence base and about barriers to implementation; and feasibility of further SDD research (e.g. likely participation rates). Results : (1) The two case studies identified complexity in the interplay of clinical and behavioural components of SDD, involving multiple staff. However, from the perspective of individual staff, delivery of SDD was regarded as simple and straightforward. (2) The Delphi study (n = 42) identified (a) specific barriers to SDD implementation, (b) uncertainty about the evidence base and (c) bimodal distributions for key variables, e.g. support for, or opposition to, SDD. (3) The national survey (n = 468) identified uncertainty about the effect of SDD on antimicrobial resistance, infection rates, mortality and cost-effectiveness. Most participants would participate in further SDD research. (4) The triallist interviews (n = 10) focused largely on the substantial challenges of conducting a large, multinational clinical effectiveness trial. Conclusions : There was considerable uncertainty about possible benefits and harms of SDD. Further large-scale clinical effectiveness trials of SDD in ICUs may be required to address these uncertainties, especially relating to antimicrobial resistance. There was a general willingness to participate in a future effectiveness RCT of SDD. However, support was not unanimous. Future research should address the barriers to acceptance and participation in any trial. There was some, but a low level of, interest in adoption of SDD, or studies to encourage implementation of SDD into practice

    The impact of Covid-19 pandemic on hospices : a systematic integrated review and synthesis of recommendations for policy and practice

    Background: The Covid-19 pandemic resulted in the development of numerous recommendations for practice and policy for specialist palliative care provided by hospices in United Kingdom (UK), as hospices were significantly affected by the pandemic and protections put in place. The aim of this review is to identify and synthesise recommendations or implications for policy and practice that have been generated for adult hospice specialist palliative care during the first 24 months of the Covid-19 pandemic. Methods: AMED, BNI, CINAHL, EMBASE, EMCARE, HMIC, Medline, PsycINFO, PubMed databases were searched for peer-reviewed papers, as well as hand searchers for grey literature. Literature relating to hospices and Covid-19 in the UK were included and a thematic synthesis of recommendations for hospice policy and practice was undertaken. Results: 858 articles were identified with 12 meeting the inclusion criteria. Fifty-eight recommendations or implications were identified: 31 for policy, 27 for practice, and 10 covering both. Recommendations were organised under ten themes. There were several recommendations seeking to secure hospice resources to mitigate the short-term impact of the pandemic, as well as those focused on longer-term implications such as core funding. The impact of the pandemic on the quality of hospice care was the focus for numerous recommendations around improving integration of hospice care in the community, provision of bereavement support and better use of Advance Care Plans (ACP). However, there were significant gaps related to carer visitation in hospices, inequities of palliative care, or hospice-at-home services. Conclusion: The Covid-19 pandemic and protections exposed several ongoing policy and practice needs, especially around hospice resources, while generating novel issues for hospices to address. Significant policy gaps remain to be addressed to mitigate the impact of the pandemic on the quality of hospice specialist palliative care

    BRCA2 polymorphic stop codon K3326X and the risk of breast, prostate, and ovarian cancers

    Background: The K3326X variant in BRCA2 (BRCA2*c.9976A>T; p.Lys3326*; rs11571833) has been found to be associated with small increased risks of breast cancer. However, it is not clear to what extent linkage disequilibrium with fully pathogenic mutations might account for this association. There is scant information about the effect of K3326X in other hormone-related cancers. Methods: Using weighted logistic regression, we analyzed data from the large iCOGS study including 76 637 cancer case patients and 83 796 control patients to estimate odds ratios (ORw) and 95% confidence intervals (CIs) for K3326X variant carriers in relation to breast, ovarian, and prostate cancer risks, with weights defined as probability of not having a pathogenic BRCA2 variant. Using Cox proportional hazards modeling, we also examined the associations of K3326X with breast and ovarian cancer risks among 7183 BRCA1 variant carriers. All statistical tests were two-sided. Results: The K3326X variant was associated with breast (ORw = 1.28, 95% CI = 1.17 to 1.40, P = 5.9x10- 6) and invasive ovarian cancer (ORw = 1.26, 95% CI = 1.10 to 1.43, P = 3.8x10-3). These associations were stronger for serous ovarian cancer and for estrogen receptor–negative breast cancer (ORw = 1.46, 95% CI = 1.2 to 1.70, P = 3.4x10-5 and ORw = 1.50, 95% CI = 1.28 to 1.76, P = 4.1x10-5, respectively). For BRCA1 mutation carriers, there was a statistically significant inverse association of the K3326X variant with risk of ovarian cancer (HR = 0.43, 95% CI = 0.22 to 0.84, P = .013) but no association with breast cancer. No association with prostate cancer was observed. Conclusions: Our study provides evidence that the K3326X variant is associated with risk of developing breast and ovarian cancers independent of other pathogenic variants in BRCA2. Further studies are needed to determine the biological mechanism of action responsible for these associations

    Clinical parameters affecting survival outcomes in patients with low-grade serous ovarian carcinoma: An international multicentre analysis

    Background: Women with low-grade ovarian serous carcinoma (LGSC) benefit from surgical treatment; however, the role of chemotherapy is controversial. We examined an international database through the Ovarian Cancer Association Consortium to identify factors that affect survival in LGSC. Methods: We performed a retrospective cohort analysis of patients with LGSC who had had primary surgery and had overall survival data available. We performed univariate and multivariate analyses of progression-free survival and overall survival, and generated Kaplan–Meier survival curves. Results: Of the 707 patients with LGSC, 680 (96.2%) had available overall survival data. The patients’ median age overall was 54 years. Of the 659 patients with International Federation of Obstetrics and Gynecology stage data, 156 (23.7%) had stage I disease, 64 (9.7%) had stage II, 395 (59.9%) had stage III, and 44 (6.7%) had stage IV. Of the 377 patients with surgical data, 200 (53.0%) had no visible residual disease. Of the 361 patients with chemotherapy data, 330 (91.4%) received first-line platinum-based chemotherapy. The median follow-up duration was 5.0 years. The median progression-free survival and overall survival were 43.2 months and 110.4 months, respectively. Multivariate analysis indicated a statistically significant impact of stage and residual disease on progression-free survival and overall survival. Platinum-based chemotherapy was not associated with a survival advantage. Conclusion: This multicentre analysis indicates that complete surgical cytoreduction to no visible residual disease has the most impact on improved survival in LGSC.     E-cadherin breast tumor expression, risk factors and survival : Pooled analysis of 5,933 cases from 12 studies in the Breast Cancer Association Consortium

    E-cadherin (CDH1) is a putative tumor suppressor gene implicated in breast carcinogenesis. Yet, whether risk factors or survival differ by E-cadherin tumor expression is unclear. We evaluated E-cadherin tumor immunohistochemistry expression using tissue microarrays of 5,933 female invasive breast cancers from 12 studies from the Breast Cancer Consortium. H-scores were calculated and case-case odds ratios (OR) and 95% confidence intervals (CIs) were estimated using logistic regression. Survival analyses were performed using Cox regression models. All analyses were stratified by estrogen receptor (ER) status and histologic subtype. E-cadherin low cases (N = 1191, 20%) were more frequently of lobular histology, low grade, > 2 cm, and HER2-negative. Loss of E-cadherin expression (score <100) was associated with menopausal hormone use among ER-positive tumors (ever compared to never users, OR = 1.24, 95% CI = 0.97-1.59), which was stronger when we evaluated complete loss of E-cadherin (i.e. H-score = 0), OR = 1.57, 95% CI = 1.06-2.33. Breast cancer specific mortality was unrelated to E-cadherin expression in multivariable models. E-cadherin low expression is associated with lobular histology, tumor characteristics and menopausal hormone use, with no evidence of an association with breast cancer specific survival. These data support loss of E-cadherin expression as an important marker of tumor subtypes.Peer reviewe
